By completing your wellness profile, you gain access to a customized health portal which allows you to: print detailed wellness profile results, participate in the program's health challenges, start an activity log, build your own workout, read helpful health articles, and so much more.

What is a wellness profile?

A wellness profile is an online review of your lifestyle choices and health status. It is a personalized health assessment — available to you free of charge — that helps you identify potential health risks so that you can take steps to avoid them.

Why should I do an annual wellness profile?

By examining your health-related behavior and health history, your wellness profile can give you more positive control over your health and wellness choices. When repeated annually, the wellness profile results can reveal changes in your overall health over time.

My wellness profile asks for some test results. How do I obtain them?

You can get the information you need by getting a cardiac risk profile (CRP). The CRP is the first step in the wellness profile journey to better health. If you are a federal employee, the CRP may be available to you at no cost to you at your worksite health center. Or, you can get a CRP from your health care provider.

A CRP provides you with blood pressure, cholesterol, glucose (blood sugar), height, and weight measurements. This information helps identify some of the factors that may put you at risk for heart disease — and can be entered into your wellness profile to give an even clearer picture of your health status. You may also add results from blood pressure or blood cholesterol screenings from recent doctor or hospital visits to your wellness profile. However, if you do not yet have that information, you should still fill out the wellness profile — you can just leave those questions blank and go back to complete them when you have the information.

How is my wellness profile information protected?

Your wellness profile data is protected by layers of strict access controls and security safeguards. Federal Occupational Health (FOH) worksite staff, employer staff, and employee management cannot view your individual information. Intentional unauthorized access of personal or health information would result in severe administrative action against any offender(s). For additional security, your information is securely stored in an independently audited data center that meets rigorous federal security standards. This high-security center is continuously monitored for physical threats or system intrusion to help ensure your data is kept safe.

What kinds of questions are on the wellness profile?

The wellness profile asks questions about your eating habits, exercise habits, and perceptions of your health. You can download a sample wellness profile, if you would like.

What happens after I take my wellness profile?

Upon completing your wellness profile, you will receive a summary report explaining your risk levels for each wellness category, as well as access to a wealth of tools and resources to take you even further on your personal path to better health.
